First, find a nice image. I used this one of Emma Watson.
What I find really important in icon-making is a good crop. So play with your image a bit until you think you've got the best crop.
I cropped mine like this.
The image is too dark, so duplicate the base and set it to Screen 43%. I only used one screen layer, but for darker images you may need a couple more.
Next, duplicate the base layer again, drag it to the top, and set it to Soft Light 100%.
Now go to Layer > New Adjustment Layer > Brightness/Contrast. Each image is different, so you'll most likely need to use different settings, but I put in these.
Make a new layer and fill it with #100E2C. Set it to Exclusion 74%.
Go to Layer > New Adjustment Layer > Hue/Saturation. ONLY move the middle bar that says Saturation. I used these settings.
Use this texture by ewanism and set it to Multiply 41%.
Then, I used this light texture and set it to Screen 100%. I moved it around a little until looked right and ended up with this.
Use this texture and set it to Multiply 26%.
This is optional. Pick a font, preferably a very simple one. I used Verdana 8 pt. Add whatever text you want, I just used 'daydream.' Move it around until it looks nice.
Now, make a new layer. Take the Rectangle Marquee Tool and make a rectangle around the area of the text. Fill it with #FFFFFF (white).
